Abdominal cocoon or sclerosing encapsulating peritonitis is a rare condition presenting as subacute or acute intestinal obstruction resulting from encasement of bowel by a dense fibrotic membrane, partially or completely, up to variable lengths. Pre-operative diagnosis is difficult. A young female presenting with recurrent subacute intestinal obstruction revealed multiple air-fluid levels in her abdomen radiograph. Upon laparotomy, she was found to be suffering from idiopathic abdominal cocoon. Decocooning was done. She had an uneventful post-operative recovery.
